Rural Equity for Advancing Community Health
The Hawaii Rural Equity for Advancing Community Health (REACH) program is an evidence-based community education and outreach program that expands access to knowledge of telehealth services for four rural and distressed communities on the Big Island of Hawaii.
These communities are targeted for provision of resources including health screening services (blood pressure, pulse, oxygenation, glucose, cholesterol), digital literacy education, and basic telehealth skills training. Communities will be assessed through in-person/phone interviews and data driven surveys to determine the ability of the residents to participate in telehealth.

Goals & Objectives
The goal of the REACH program grant is to increase understanding of COVID prevention and expand
utilization of telehealth technologies in order to improve health in four distressed, rural, and
underserved communities.
HI-TEK Program
The HI-TEK Program aims to enhance the telehealth experience by providing participants with essential tools and resources to monitor their health from the comfort of their homes. Through this pilot, we hope to better understand the needs of our community members and refine our approach to delivering telehealth services effectively.
This program aims provides free health monitoring tools and telehealth training. Qualifying participants will receive personalized training and support to monitor their health from home using smart devices like blood pressure monitors, glucose meters, and more.
